HIV Early Intervention
The Early Intervention Services (EIS) Program offers coordinated care for people living with or at risk for HIV, focused on early access, prevention, and ongoing support.
Services include:
- Primary HIV Medical Care – HIV diagnosis and treatment, routine checkups and labs, care for STIs and Hepatitis C, and help accessing HIV medications and insurance or ADAP.
- Early Intervention & Testing – HIV, Hepatitis C, and STI testing (routine and rapid), prevention counseling, PrEP/PEP, and immediate linkage to care for newly diagnosed or returning patients.
- Care Coordination & Support– Medical case management, benefits and insurance assistance, referrals to housing, mental health, and substance use services, and help overcoming barriers to care.
- Health Education – One‑on‑one and group education on HIV prevention, safer sex, harm reduction, and Hepatitis A, B, and C information and vaccines.
- Telehealth & Sensitive Services – In‑person and telehealth visits, including audio‑only options for sensitive services such as HIV testing, STI care, and pregnancy‑related concerns.
- Community Partnerships – Collaboration with local health providers, prevention programs, and community organizations to ensure seamless, high‑quality care.
